Dawn Marie Photography Model Release Form

In consideration of my engagement as a model, upon the terms herewith stated, I hereby give Dawn Marie Photography [photographer], her heirs, legal representatives and assigns, those for whom the photographer is acting, and those acting with her authority and authority, of the specific photographic images (“Images”) of me [Model] that Photographer created and made has my permission.

I waive any right to inspect or approve the finished Images, or to inspect or approve any version of any use(s) to which the Images may be employed or used in conjunction therewith.

I release and discharge Photographer and those acting under his authority from any and all liabilities, claims and demands arising out of or relating to any blurring, distortion, or alteration, whether intentional or otherwise, that may occur or be produced in connection with Image, or in connection with any processing, alteration, transmission, display, or publication of Images.

I represent that I am not affiliated with any model agency and have made no other agreement, whether written or oral, pertaining to the creation, use or publication of any Images.

If Photographer, in her sole discretion, provides me with Images, and I display them on the Internet, I agree to credit Photographer's work by displaying @dawnmariephotography.716 or the dawnmariephotography watermark on the images, that watermark to be provided to me by Photographer in electronic form suitable for display.

I acknowledge that I have read this release and understand the terms and conditions in it. I further acknowledge that Photographer, as defined herein, relies on the accuracy of my representations.
